Introductie van de MiniMax-studie: een prospectief gerandomiseerde multicentrumstudie naar de behandeling van gedisloceerde intra-articulaire distale radiusfracturen bij oudere patiënten

Samenvatting

Achtergrond. Gedisloceerde intra-articulaire fracturen van de distale radius zijn met name bij de oudere patiënt een frequent en toenemend klinisch probleem. Voor deze fracturen is chirurgische behandeling gewenst om optimaal functioneel herstel te bewerkstelligen. Op dit moment bestaat geen consensus betreffende de chirurgische techniek. Open repositie en interne fixatie met een hoekstabiele volaire plaatosteosynthese lijkt een veelbelovend alternatief voor de veelal toegepaste gesloten repositie en externe fixatie of interne fixatie via de dorsale benadering.

Studieopzet. Patiënten met een gedisloceerde intra-articulaire distale radiusfrac-tuur en leeftijd boven de 50 jaar zullen worden gevraagd te participeren in een prospectief gerandomiseerde multicentrumstudie. Twee behandelmethoden worden toegepast: open repositie en volaire fixatie met hoekstabiele plaatosteosynthese of gesloten repositie en externe fixatie in combinatie met alle stabili-teitsverhogende technieken zoals K-draden, een percutane schroef, bottrans-plantatie en/of osteoconductief cement. Beide technieken hebben als doel maximale stabiliteit te bieden in de fractuurdelen, waarbij het verschil in uitkomst wellicht te wijten is aan de open of gesloten techniek.

Primaire uitkomsten. Functioneel herstel in het eerste jaar, pijnbeleving en patiëntentevredenheid.

Secundaire uitkomsten. Radiologische parameters en het optreden van complicaties. Analyse van 148 patiënten zal plaatsvinden na behandeling volgens het ‘intention to treat’-principe om een verschil van vijf punten op de functionele dash-score aan te tonen.

Discussie. Deze studie zal uitwijzen of de behandeling van gedisloceerde intra-articulaire distale radiusfracturen met een hoekstabiele volaire fixatie in het eerste jaar vergelijkbare of betere functionele uitkomsten geeft dan de minimaal invasieve techniek.

Studieregistratie. De Medisch Ethische Toetsingscommissie Zuidwest Holland gaf een positief oordeel wat betreft de uitvoering van de MiniMax-studie. Tevens werd bij de lokale Medisch Ethische Toetsingscommissies van de deelnemende ziekenhuizen toestemming verkregen voor lokale uitvoerbaarheid.
