Geïsoleerde scafoïddislocatie met axiale carpale dissociatie

Samenvatting

Wij presenteren een casus van een patiënt met een geïsoleerde scafoïddislocatie met axiale carpale dissociatie. De diagnostiek en behandeling van deze zeldzame entiteit worden kort besproken. Daarbij ligt nadruk op het belang van open repositie, interne fixatie en ligamentreconstructie in de acute fase. Vooral herstel van het scafolunaire ligament is van belang. Op deze wijze wordt het instabiele radiale gedeelte van de carpalia aan het stabiele ulnaire gedeelte gefixeerd. De huidige behandelstrategie bepleit ligamentreconstructie in de acute fase om de normale anatomie te herstellen om zowel op korte als op lange termijn een goede functie te behouden. De prognose is goed, mits er voor deze behandeling wordt gekozen.
