8. Immunologische afweer in speeksel

Samenvatting

  • De immuunglobulinen in speeksel behoren voornamelijk tot de IgA-klasse en zijn vooral als secretie-IgA aanwezig (s-IgA): een complex van twee IgA-moleculen, een J-keten en een secretory component.
  • Van de IgA-moleculen in speeksel wordt 98% in de speekselklieren gesproduceerd, slechts 2% is afkomstig uit serum.
  • De hoogste s-IgA-concentratie wordt aangetroffen in mukeus labiaal speeksel. De labiale speekselkliertjes leveren ongeveer 35% van het s-IgA in totaalspeeksel.
  • In speeksel behoort ongeveer 60% van de s-IgA-moleculen tot de IgA1-subklasse. IgA1-moleculen worden onder invloed van proteolytische enzymen gemakkelijker afgebroken dan IgA2-moleculen.
  • De belangrijkste functie van s-IgA is het beschermen van de mucosale membranen tegen kolonisatie van micro-organismen. Dit geschiedt mogelijk in samenwerking met andere speekselcomponenten, zoals lactoferrine, agglutinine en mucinen.
  • Patiënten die onvoldoende s-IgA hebben, vertonen een hogere cariësscore, terwijl daarnaast S. mutans in hogere frequentie aanwezig is.
