Hormonale en reproductieve factoren in de ontwikkeling van blaaskanker

Samenvatting

Bij mannen wordt drie tot vier keer vaker blaaskanker vastgesteld dan bij vrouwen. Historische verschillen in rookgedrag en beroepsblootstelling verklaren de hogere incidentie bij mannen niet geheel. Er zijn aanwijzingen voor hormonale factoren in de etiologie van blaaskanker. In-vitrostudies en diermodellen suggereren dat de androgeenreceptor een essentiële rol speelt bij de carcinogenese en progressie van blaaskanker. Soortgelijke studies naar de invloed van oestrogenen en de oestrogeenreceptor leiden tot tegenstrijdige resultaten en naar de rol van progesteron en de progesteronreceptor is nog weinig onderzoek gedaan. Epidemiologische studies naar reproductieve en hormonale factoren laten vaak inconsistente resultaten zien. Echter, grote patiëntcontrole- en cohortonderzoeken laten consistent zien dat pariteit het risico op blaaskanker verlaagt. Hormoontherapie met oestrogenen in combinatie met progesteron lijkt het risico ook te verlagen. Verdere onderzoeken zijn nodig om te bepalen hoe geslachtshormonen de tumorontwikkeling in het urotheel kunnen beïnvloeden.
