Het langetermijnbeloop van depressie bij patiënten in de open bevolking en de huisartspraktijk

Een systematische review

Samenvatting

Achtergrond De huidige kennis over het langetermijnbeloop van depressie is met name gebaseerd op onderzoeksresultaten in de tweede lijn. Kennis over het langetermijnbeloop in minder geselecteerde populaties is onmisbaar om in de eerste lijn verantwoorde beleidskeuzes te kunnen maken.

Methoden Literatuuronderzoek. Onderzoeken gepubliceerd tussen 1970 en 1999, met een follow-up van ten minste 5 jaar werden geïncludeerd. Een nieuwe search in 2001 leverde geen nieuwe artikelen op. Gegevens over recidieven, terugval, psychopathologie, functionele status en kwaliteit van leven werden bestudeerd.

Resultaten Acht onderzoeken voldeden aan de insluitingscriteria. De gevonden recidiefpercentages of percentages van depressie tijdens de vervolgmetingen varieerden tussen de 30 en 40%. In jongere en oudere leeftijdsgroepen werd een hoger recidief-percentage gevonden.

Conclusies De resultaten uit de beschikbare onderzoeken waren moeilijk te vergelijken vanwege de grote verschillen in populaties en methoden. Desondanks lijkt depressie in de open bevolking en huisartspraktijk een duidelijk gunstiger langetermijn-prognose te kennen dan in de psychiatrie.
