Evaluatie en bespreking van de kwaliteit van leven van adolescenten met type-1-diabetes

Effect op lichamelijk en psychisch welbevinden, tevredenheid met de zorg en glykemische instelling

Samenvatting

Doel. Het huidige onderzoek beoogt na te gaan wat de effecten zijn van het regelmatig in kaart brengen en bespreken van de ervaren kwaliteit van leven (KvL) van adolescenten met diabetes als onderdeel van de periodieke controle op het psychosociaal welbevinden, tevredenheid met de zorg en glykemische instelling. Patiënten. Vanuit vier kinderdiabetespoliklinieken werden 91 adolescenten en hun ouders geïncludeerd. De klinieken werden gerandomiseerd over interventie- en controleconditie, voor drie opeenvolgende consulten.
Methode. De interventie bestond uit het evalueren van de KvL van de adolescenten (met de PedsQL) en het bespreken hiervan met de kinderarts. Naast het HbA1c werden vragenlijsten (lichamelijk en psychosociale gezondheid: CHQ, depressie: CES-D, gezinsconflicten: DFCS, tevredenheid met de zorg: PEQ-D) afgenomen bij de start en na afloop van de studie om veranderingen in psychosociaal welbevinden, diabetesregulatie en tevredenheid met de zorg te testen.
Resultaten. Gemiddelde leeftijd was 14,9 jaar (SD ± 1,1). Gemiddeld HbA1c bij aanvang was 8,8% (±1,7). De interventiegroep verbeterde op de psychosociale gezondheid somschaal (p=0,006). Deze verbetering vertoonde een interactie met HbA1c. Adolescenten met hoger HbA1c profiteerden minder van de interventie dan beter ingestelde patiënten. De interventie had geen effect op glykemische instelling (p=0,54); echter, de interventiegroep was na afloop van de studie tevredener met de zorg (p=0,009).
Discussie. Het evalueren van de KvL heeft een positief effect op het psychosociaal welbevinden van de adolescenten. Adolescenten met een slechte glykemische instelling profiteren echter minder. De interventie wordt goed gewaardeerd, maar een effect op HbA1c kon niet worden aangetoond.
