Epstein-barr-virusinfectie: kliniek en trapsgewijze diagnostiek

Samenvatting

De klinische manifestatie van een acute infectie door het epstein-barr-virus (EBV), gekenmerkt door de klassieke trias van koorts, faryngitis en lymfadenopathie, wordt de ziekte van Pfeiffer genoemd. Bij jonge kinderen wordt meestal een aspecifiek beeld gezien, hoewel bij deze groep ook het klassieke beeld gezien kan worden, zoals de casus illustreert. In dit artikel worden de verschillende klinische beelden en complicaties van EBV-infectie bij kinderen belicht. De diagnostiek naar EBV kan trapsgewijs worden verricht. In de eerste stap worden de EBNA-antistoffen bepaald, waarbij een hoog-positieve uitslag een actieve EBV-infectie onwaarschijnlijk maakt. Bij een negatieve uitslag worden als tweede stap de VCA IgM- en IgG-antistoffen bepaald.
