Diagnostiek van longembolie

Wanneer de huisarts bij een patiënt longembolie vermoedt, kan hij eigenlijk niets anders doen dan verwijzen naar de tweede lijn. Dit is vaak nogal onbevredigend, omdat uiteindelijk slechts 10- 20% van de verwezen patiënten de diagnose longembolie krijgt. In de tweede lijn bleek het mogelijk om longembolie veilig uit te sluiten met behulp van de Wells-beslisregel en een D-dimeertest. Daarom voerden we een prospectief onderzoek uit om eenzelfde strategie te valideren voor de eerste lijn (AMUSE-2). Huisartsen in heel Nederland includeerden 598 patiënten bij wie zij een longembolie vermoedden. De huisartsen pasten de Wells-beslisregel toe en voerden een point of care D-dimeertest uit. Alle patiënten kregen vervolgens volgens protocol een verwijzing naar de tweede lijn. Bij 73 patiënten (prevalentie 12,2%) stelde men de diagnose longembolie. Bij 272 van de 598 patiënten (45%) was er sprake van een negatieve Wells-beslisregel (score ≤ 4) en een negatieve kwalitatieve D-dimeertest. Slecht 4 van de 272 patiënten kregen de diagnose longembolie (failure rate 1,5%; 95%-BI 0,4-3,7%), hetgeen niet verschilde van een negatieve CT-scan. De conclusie is dat de huisarts longembolie veilig kan uitsluiten aan de hand van de Wells-beslisregel en een kwalitatieve D-dimeertest.
