Abstract

Hartfalen is een klinisch syndroom dat wordt gekenmerkt door tekortschieten van de pompwerking van het hart, leidend tot een complex van klachten en verschijnselen. Om de tekortschietende circulatie op te vangen treden allerlei adaptatiemechanismen in werking, zoals activitatie van het renine-angiotensine-aldosteronsysteem en stijging van de natriuretische peptiden. Als bij gedegen anamnese en lichamelijk onderzoek de verdenking op de aanwezigheid van hartfalen ontstaat, is objectief bewijs van cardiale disfunctie (beeldvorming) nodig om het vermoeden van hartfalen te onderbouwen. Een verhoogde waarde van het natriuretisch peptide type B (BNP) ondersteunt de diagnose, maar is onvoldoende om de diagnose hartfalen te stellen. Bij de diagnostiek gaat het er overigens niet alleen om de correcte diagnose te stellen, maar ook om de oorzaak te achterhalen en om een indruk te krijgen van de ernst en de prognose. Al deze gegevens zijn van belang om de behandeling van hartfalen te optimaliseren.
