Ontrafeling van de pathofysiologie: wat zijn de therapeutische consequenties?

Het complex regionaal pijnsyndroom (crps), ook wel bekend onder de naam posttraumatische dystrofie en/of sudeckdystrofie, is een ziektebeeld dat optreedt als complicatie na weefselbeschadiging door trauma of chirurgie. Ondanks het feit dat het ziektebeeld al meer dan honderd jaar geleden door Sudeck beschreven is1, was over de pathofysiologie tot voor kort relatief weinig bekend. Aan de hand van recente onderzoeksgegevens lijkt de puzzel zich enigszins te ontrafelen. In deze bijdrage gaan wij in op nieuwe bewijzen voor een steriele ontsteking, en formuleren we een hypothese over hoe bij hetcrps een inflammatoir proces kan leiden tot centrale sensitisatie. Vervolgens staan wij kort stil bij de therapeutische consequenties die hieruit voortkomen. Duidelijk is dat nog veel aanvullend onderzoek nodig is.
