Comorbiditeit van alcoholverslaving en een angst- of stemmingsstoornis

Welke zorg wordt geboden en hoe kan preventie worden vormgegeven?

Opzet:

De meest voorkomende psychische stoornissen onder de Nederlandse bevolking zijn angst-, stemmings- en middelenstoornissen. Ook comorbiditeit van deze stoornissen komt veel voor. Omdat er geruime tijd zit tussen het ontstaan van een alcoholstoornis en een comorbide angst- of stemmingsstoornis biedt dit mogelijkheden voor preventie. Wij hebben onderzocht welke behandelingen en preventieve interventies beschikbaar zijn, hoeveel mensen met een comorbide stoornis gebruik maken van zorg en wat er in de Nederlandse zorgpraktijken gedaan wordt aan preventie van een comorbide alcoholstoornis en een angst- of stemmingsstoornis. Wij voerden hiertoe een literatuur- en praktijkonderzoek uit. Het praktijkonderzoek bestond uit een online vragenlijst onder leidinggevenden in de GGZ- en verslavingspreventie. In totaal werden 27 van de 53 vragenlijsten volledig ingevuld (39%).

Resultaten:

Verschillende behandelingen zijn beschikbaar voor een alcoholstoornis met een comorbide angst- of stemmingsstoornis en in Nederland zijn daarvoor richtlijnen opgesteld. Voor preventie is vroegtijdige herkenning noodzakelijk, maar in Nederlandse zorgpraktijken wordt een tweede comorbide stoornis vaak niet herkend. Een interventie om deze comorbide stoornis te voorkomen is bovendien niet aanwezig.

Conclusie:

De GGZ- en verslavingspreventie hebben steeds meer aandacht voor comorbiditeit, maar gestandaardiseerde interventies en gestructureerde samenwerking voor preventie van comorbiditeit ontbreken. Deskundigheidsbevordering, screening op risicofactoren of met behulp van online tests en meer publieke bekendheid zijn nodig om comorbiditeit te kunnen voorkomen.
