Chronische pijn bij dementie en bij aandoeningen met een verhoogd risico op cognitieve achteruitgang

samenvatting

De kans op het krijgen van chronische pijnklachten én op het ontstaan van dementie neemt toe naarmate men ouder wordt. Dit betekent dat door de toenemende vergrijzing het aantal demente ouderen met chronische pijn eveneens sterk zal toenemen. De vraag rijst of en op welke wijze demente ouderen pijn ervaren. Het tekortschieten van de pijndiagnostiek, vooral in een gevorderd stadium van dementie, wordt weerspiegeld in een verminderd gebruik van pijnstillers door ouderen met een dementie. Inzicht in mogelijke veranderingen in pijnbeleving zoals die worden waargenomen in de slechts enkele klinische studies, kan vergroot worden door kennis over de neuropathologie die per subtype dementie kan verschillen. Het is opvallend dat er helemaal geen studies zijn verricht naar pijnbeleving bij aandoeningen van het centrale zenuwstelsel met een verhoogd risico op cognitieve achteruitgang zoals de ziekte van Parkinson en multiple sclerose. Bij deze ziekten behoort pijn tot één van de klinische symptomen en het is tot dusver onbekend of er veranderingen in pijnbeleving optreden in een fase waarin het cognitief functioneren achteruitgaat. Tenslotte worden kort een aantal instrumenten besproken welke het meest geschikt zijn om pijn te diagnostiseren bij communicatieve en non-communicatieve patiënten met een dementie.
