9 Brachytherapie

Samenvatting

Inhoud – 1 Inleiding – 2 Fysische en biologische aspecten van brachytherapie – 3 Klinische toepassingen – 4 Kwaliteitscontrole – Brachytherapie is een vorm van inwendige bestraling, waarbij radioactieve bronnen met applicatoren direct in of dicht tegen het tumorweefsel geplaatst worden. Kenmerkend zijn een hoge dosis in de tumor en een relatief lage dosis in omliggende organen door de steile dosisgradiënt. De radioactieve bronnen kunnen verschillende dosistempi hebben, hetgeen mede bepaalt of de bronnen permanent of tijdelijk in de patiënt geïmplanteerd worden. Andere kenmerken die bepalen of een bron geschikt is voor brachytherapie zijn de halveringstijd, het soort en de energie van de uitgezonden straling en de specifieke activiteit. Brachytherapie wordt onder andere toegepast bij gynaecologische en urologische tumoren en tumoren uitgaande van de tractus digestivus, mamma en huid. Voor reconstructie van de applicator en dosisplanning wordt US, CT of MRI gebruikt. Om de kwaliteit van de brachytherapiebehandeling te garanderen, worden de nauwkeurigheid van de dosisafgifte, de werking van de apparatuur, de applicatoren en het planningssysteem regelmatig getest.
