Bekkenfysiotherapie bij onvrijwillig urineverlies

Bekkenfysiotherapie voor urine-incontinentie wordt vaak genoemd als behandeling van eerste keuze.

Bij stressincontinentie treedt onwillekeurig urineverlies op bij fysieke inspanning: de druk in de blaas overschrijdt de maximale druk in de plasbuis in afwezigheid van activiteit van de blaasspier. Ter bevordering van het extrinsiek afsluitmechanisme van de plasbuis richt bekkenfysiotherapie zich bij stressincontinentie vooral op krachtsverbetering en coördinatie van de periurethrale en bekkenbodemspieren. Behandelmethoden zijn: oefentherapie met of zonder biofeedback, elektrostimulatie en vaginale kegeltjes. Met name bekkenbodemspieroefeningen zijn effectief. Overactiviteit van de blaas is een disfunctie van de blaas waarbij de patiënt geen of onvoldoende controle heeft over plotselinge, onwillekeurige contracties van de blaasspier, waardoor vroegtijdig urine verloren kan gaan. Bekkenfysiotherapie richt zich op een reductie of opheffing van deze blaasoveractiviteit via reflexinhibitie. De behandeling bestaat uit informatie en voorlichting, elektrostimulatie, blaastraining of gedragstherapie en/of oefentherapie met of zonder biofeedback. Vooral elektrostimulatie blijkt een werkzame behandelvorm.

Radicale prostatectomie is de voornaamste oorzaak van onvrijwillig urineverlies bij mannen. Bekkenbodemeducatie als nabehandeling vermindert de duur en de mate van incontinentie. Bekkenfysiotherapie blijkt in veel gevallen bij urine-incontinentie een effectieve behandeloptie.
