40 Rectaal bloedverlies

Samenvatting

Rectaal bloedverlies is in de Nederlandse bevolking een veelvoorkomend probleem, waardoor vele artsen ermee te maken krijgen. Het is dus van belang om te weten wat de verschillende oorzaken kunnen zijn. Het is de taak van de arts om stapsgewijs een juiste differentiële diagnose op te stellen.
Het is belangrijk om te weten wanneer een patiënt moet worden doorverwezen voor aanvullend onderzoek. Bij acuut rectaal bloedverlies kan een patiënt door het vele bloedverlies in een hemodynamische shock raken. Deze patiënten dienen dus met spoed ingestuurd te worden. De patiënten met chronisch bloedverlies hoeven meestal niet doorverwezen te worden. Een aantal minder ernstige oorzaken, zoals hemorroïden, kan de arts behandelen. Pas op voor de valkuil dat de hemorroïd te snel als de (enige) verklaring wordt beschouwd: blijf alert op de mogelijkheid van een tweede oorzaak, namelijk een rectumcarcinoom.
Bij sommige patiënten wordt geen oorzaak gevonden en dus geen diagnose gesteld. Als de patiënt geen symptomen heeft die zouden kunnen wijzen op een maligniteit, kan de arts een afwachtend beleid instellen. Het is dan wel van belang om duidelijke richtlijnen mee te geven aan de patiënt, zodat deze weet bij welke klachten een nieuw bezoek aan de arts nodig is.
Bij voor een maligniteit verdachte symptomen is nader onderzoek gewenst. Alle patiënten met rectaal bloedverlies die in een van de risicogroepen vallen voor colorectaal kanker dienen zonder meer naar de specialist verwezen te worden voor aanvullend onderzoek. Uit verschillende onderzoeken blijkt dat een vroege diagnose bij colorectaal kanker een betere prognose betekent.4,10,17,19,50,77 Een arts moet dus zeker zijn van zijn zaak wanneer hij besluit een patiënt niet voor nader onderzoek door te sturen. Daartegenover staat dat een ongericht verwijsbeleid leidt tot een te grote belasting van de tweede lijn. De kans op het hebben van een maligniteit is kleiner indien:
  • de patiënt jonger is dan 45 jaar;
  • er geen eerstegraads familielid is met een colorectaal carcinoom (ofwel presenterend vóór het 50e levensjaar, ofwel twee of meer in één familie, ongeacht de leeftijd);
  • de patiënt niet uit een familie met FAP of het syndroom van Lynch komt;
  • er geen coloncarcinoom of adenomateuze poliepen in de voorgeschiedenis aanwezig zijn.
