30 Bovenbuikklachten, niet-acute

Samenvatting

In de algemene populatie zijn niet-acute bovenbuikklachten een veelvoorkomend verschijnsel. Slechts een klein gedeelte van de mensen met deze klachten wendt zich tot een arts. Ongerustheid speelt hierbij een grote rol. Ernstige aandoeningen liggen zelden aan deze klachten ten grondslag, maar zijn wel het eerste waarop de arts zich door middel van alarmsymptomen oriënteert. De prognose van een maligniteit in slokdarm en maag blijft ondanks toegenomen diagnostische mogelijkheden matig tot slecht. Bij afwezigheid van alarmsymptomen is een gedetailleerde anamnese het belangrijkste instrument voor bepaling van een adequaat differentiaaldiagnostisch beleid. Het natuurlijk beloop van deze klachten is gunstig, maar er zijn enkele aandoeningen waarvoor interventie nodig en nuttig is. Een door H. pylori veroorzaakt peptisch ulcus in maag of duodenum moet bijvoorbeeld, gezien de therapeutische consequenties, als zodanig worden gediagnosticeerd. Derhalve zal bij persisteren van de klachten worden overgegaan tot nadere diagnostiek, gericht op het aantonen of uitsluiten van aandoeningen met specifieke consequenties.
