3 Effectieve implementatie van verbetering in de zorg: een systematische aanpak

Samenvatting

  • Effectieve implementatie van nieuwe werkwijzen vereist een goede voorbereiding en planning, en een systematische aanpak.
  • De volgende stappen zijn belangrijk in een systematische aanpak van implementatie.
    • Formuleren van een concreet, goed onderbouwd en haalbaar voorstel voor verandering in de praktijk met duidelijke doelen.
    • In kaart brengen van de feitelijke zorg en problemen daarin.
    • Analyse van de doelgroep en de context: welke belemmerende en bevorderende factoren spelen een rol in het realiseren van de verandering?
    • Ontwikkeling en selectie van strategieën om veranderingen in te voeren: strategieën voor zowel een effectieve verspreiding als een effectieve implementatie en behoud van verandering.
    • Ontwikkeling en uitvoering van een implementatieplan met activiteiten, taken en een tijdpad.
    • Integratie van verandering in normale routines.
    • Evaluatie en (eventueel) bijstelling van het plan: continue monitoring aan de hand van indicatoren.
  • Veel implementatietrajecten verlopen niet geheel volgens deze stappen, maar ook dan biedt een planmatige aanpak houvast.
