14 Medicatieveiligheid

Samenvatting

Het in 1999 in de vs verschenen rapport ‘To err is human’, heeft laten zien dat medische fouten frequent voorkomen en dat een belangrijk deel daarvan gevormd wordt door medicatiefouten.1 Vervolgens liet het proefschrift ‘Drug safety in hospitalised patients’ zien, dat medicatiefouten ook in de Nederlandse ziekenhuizen een veelvoorkomend veiligheidsprobleem vormen.2 Alle reden dus om binnen het veiligheidsmanagementsysteem specifiek aandacht te besteden aan medicatieveiligheid.
