Wat toont een scintigrafie bij het vermoeden van een scafoïdfractuur?

Samenvatting

Inleiding: In onze instelling zijn tussen maart 2004 en september 2010 twee studies naar occulte scafoïdfracturen verricht waarin patiënten met een polstrauma werden onderzocht. In veel gevallen werden na aanvullende diagnostiek andere fracturen rondom het scafoïd waargenomen.

Doel: Het vaststellen van het aantal en de verspreiding van ten gevolge van botscintigrafie gevonden fracturen. Methode: In dit onderzoek zijn de data van 264 patiënten met een polstrauma geïncludeerd en retrospectief geanalyseerd. Initiële röntgenfoto's toonden geen afwijkingen, maar er bleef verc.denking op een scafoïdfractuur. Aanvullend werd een botscintigrafie gemaakt. Resultaten: De uitkomsten van botscintigrafie waren als volgt: 52 (19,7%) scans met een scafoïdfractuur, 90 (34,0%) met een andere fractuur in de carpale regio, 118 (44,7%) toonden geen afwijkingen en bij 4 (1,5%) bleef de diagnose onzeker.

Conclusie: Bij onderzoek naar de diagnostiek van occulte scafoïdfracturen blijkt dat op initiële röntgenfoto's onacceptabel veel andere fracturen rondom het scafoïd worden gemist.
