Verschillen in patiëntenmix tussen aios en hun opleiders

Doel

Uit eerder kleinschalig en kortlopend onderzoek is gebleken dat er mogelijk verschillen bestaan in de patiëntenmix tussen aios en opleiders. Aios zien meer kleine kwalen en minder chronische en ernstige aandoeningen dan hun opleider. Dat kan van invloed zijn op de ontwikkeling van de benodigde competenties. Wij wilden een antwoord vinden op de vraag wat de verschillen zijn in de patiëntenmix van aios huisartsgeneeskunde en hun huisartsopleiders.

Methode

Tussen maart 2008 en september 2009 extraheerden we in 73 huisartsopleidingspraktijken gedurende 6 maanden gegevens uit elektronische patiëntendossiers. De gegevens betroffen ICPC-diagnosecodes en contactfrequenties, die waren verdeeld over de leeftijdsgroepen van patiënten.

Resultaten

We includeerden 49 eerstejaars en 24 derdejaars aios en hun opleiders (32 solo-opleiders, 41 duo’s). Opleiders codeerden gemiddeld 79% van hun contacten, aios 86%. Opleiders deden ongeveer twee keer zo veel consulten per trimester, drie keer zo veel telefonische consulten en twee keer zo veel visites. Aios zagen relatief meer patiënten met oog-, huid-, oor- en luchtwegaandoeningen dan hun opleiders. Ook zagen zij relatief jongere patiënten en meer kleine kwalen. Opleiders zagen meer patiënten met cardiovasculaire ziekten, psychiatrische aandoeningen, metabole ziekten, aandoeningen van het mannelijk genitaal en sociale problematiek. Ook kregen zij meer patiënten met chronische en oncologische aandoeningen op hun spreekuur. Zowel vrouwelijke aios als vrouwelijke opleiders zagen gemiddeld bijna twee keer zoveel gynaecologische en zwangerschapsgerelateerde problematiek als hun mannelijke collega’s. Mannelijke aios en opleiders zagen gemiddeld meer aandoeningen van het mannelijk genitaal.

Conclusie

Er bestaan belangrijke verschillen tussen de patiëntenmix van aios en opleiders. Opleiders en docenten van de huisartsopleiding dienen dan ook op de hoogte te zijn van de gebieden waarop het aanbod voor aios relatief laag is. Opleiders moeten worden aangemoedigd om ervoor te zorgen dat hun aios meer chronische, complexe, psychosociale en cardiovasculaire aandoeningen zien.
