Urineverlies uit bovenbeen: een late complicatie van radiotherapie

Samenvatting

Beschreven wordt een casus waarbij een fistel is ontstaan van de blaas, naar spierloges in het rechter- en linkerbovenbeen, met als gevolg drainage van urine via een wond die is ontstaan 13 jaar na radiotherapie voor prostaatcarcinoom.
Patiënt presenteert zich met een urosepsis, hemodynamisch instabiel, op de Spoedeisende Hulp. Opname op de intensivecareafdeling is noodzakelijk. Conservatieve behandeling blijkt ontoereikend. Er wordt een urinedeviatie volgens Bricker aangelegd, waarbij de blaas in situ wordt gelaten.
