30. Transplantatie van nier, pancreas en darm

Samenvatting

Bij een niertransplantatie wordt een donornier afkomstig van een levende of overleden donor na een zo kort mogelijke bewaar- of preservatieperiode geïmplanteerd bij een patiënt met (pre)terminale nierinsufficiëntie. Bij postmortale orgaandonatie gaat het om ‘donation after brain death’ (DBD) of ‘donation after circulatory death’ (DCD). De belangrijkste chirurgische complicaties na een niertransplantatie zijn vasculaire problemen, lymfokèle en urologische problemen. Niet-chirurgische complicaties zijn infecties en afstoting. Pancreastransplantatie wordt vaak gecombineerd met niertransplantatie en kan een goede optie zijn bij een patiënt met DM type I. Darmtransplantatie kan worden uitgevoerd bij patiënten met chronisch darmfalen en zeer problematische TPV-afhankelijkheid. De verschillende chirurgische technieken worden gedetailleerd besproken, evenals de preservatie van het donororgaan.
