Spraakrevalidatie na totale laryngectomie door middel van stemprothesen

Samenvatting

Een totale laryngectomie is een operatie waarbij het volledige strottenhoofd (inclusief het hyoïd en de eerste twee trachearingen) wordt verwijderd. Hierdoor ontstaat er een defect in de voorzijde van de pharynx. Om de voedselweg weer te herstellen, worden de slijmvliesranden van het pharynxdefect in twee lagen aan elkaar vastgehecht in de mediaanlijn, en daaroverheen worden meestal ook de pharynxspieren als derde laag aan elkaar gehecht.

In de meeste Nederlandse ziekenhuizen wordt de warmte- en vochtwisselaar direct vanaf de eerste dag na de operatie gebruikt. Bij aanvang van de stemrevalidatie leert de patiënt met de vinger afsluiten op de warmte- en vochtwisselaar. Het verdient aanbeveling om in een later stadium, wanneer het stoma goed genezen is, de patiënt ook te leren hoe hij het stoma kan afsluiten met de vinger. Zo kan de patiënt wanneer het bijvoorbeeld tijdelijk niet mogelijk is een pleister te gebruiken, het stoma ook met de vinger of duim afsluiten.
