Bij een sombere stemming is het vooral van belang een onderscheid te maken tussen enerzijds de depressie, een syndroomdiagnose met specifieke criteria die betrekkelijk frequent voorkomt en een specifieke therapie vereist, en anderzijds psychosociale problemen die met een down of depressief gevoel gepaard gaan en evenmin zeldzaam zijn. Een afzonderlijke categorie vormen de patiënten met een milde chronische vorm van somberheid: dysthymie.

Het belangrijkste diagnosticum van de arts is een grondige anamnese. Het onderscheid tussen een gedeprimeerde stemming bij problemen en een depressie zit vooral in ernst en duur van de klachten. Een depressie gaat incidenteel gepaard met psychotische kenmerken (vooral wanen). Depressies afgewisseld met manische episoden treden op bij een bipolaire stoornis.

Soms staan bij een beginnende dementie depressieve symptomen op de voorgrond. Ook alcoholmisbruik en een aantal somatische aandoeningen, zoals schildklierfunctiestoornissen, een cerebrovasculair accident en de ziekte van Parkinson kunnen tot een sombere stemming leiden.
