Prikkelbaredarmsyndroom: zelfmanagement met een pocketcomputer

Doel

Vaststellen of een cognitieve gedragstherapie (CGT)-interventie met behulp van pocketcomputers haalbaar en effectief is in het kader van het zelfmanagement van prikkelbaredarmsyndroom (PDS)-patiënten.

Methode

We randomiseerden 76 PDS-patiënten; naar een controlegroep die alleen standaard huisartsenzorg ontving en een interventiegroep die daarnaast een vierweekse CGT-interventie met behulp van de pocketcomputer kreeg. Gedurende drie van de vier weken kreeg de interventiegroep situationele feedback van een psycholoog in reactie op hun antwoorden op de vragen uit het elektronische dagboek. De feedback richtte zich op signalen van inadequaat zelfmanagement: disfunctionele cognities, catastroferende gedachten en vermijdingsgedrag. Alle patiënten vulden voorafgaand aan de interventie (baseline), na de interventie (vier weken) en na drie maanden een set vragen(lijsten) in.

Resultaten

Na 4 weken rapporteerden patiënten uit de interventiegroep een significante grotere verbetering in catastroferend denken (p < 0,01), kwaliteit van leven (p < 0,05) en buikpijn (p < 0,05) dan de controlegroep. De verbetering in catastroferend denken hield ook na drie maanden stand.

Conclusie

Een CGT-interventie met behulp van pocketcomputers is haalbaar en, tenminste op de korte termijn, effectief wat betreft verbetering van buikpijn, kwaliteit van leven en catastroferend denken bij PDS-patiënten. Dat de enige langetermijnverbetering (catastroferend denken) niet bij andere symptomen was terug te zien, kan betekenen dat de verbetering in gedachten niet meteen relevant is voor een vermindering van klachten.
