4. Precisie en maatwerk – organiseren van verandering in psychiatrie en psychische gezondheidszorg

Samenvatting

In dit hoofdstuk worden de ideeën uit het eerste en tweede hoofdstuk vertaald naar de psychiatrie en de psychische gezondheidszorg. De centrale vraag hier is wat er nodig is om precisiepsychiatrie tot ontwikkeling te brengen en wat de organisatorische voorwaarden zijn om precisie en maatwerk in de zorgverlening te realiseren. Een blauwdruk heeft weinig zin omdat de omstandigheden voor iedereen net weer iets anders zijn. Het hoofdstuk stelt daarom vooral vragen waarmee zorgprofessionals, onderzoekers, managers en bestuurders aan de slag kunnen – liefst in het kader van een gezamenlijke filosofie en centrale doelstelling.

Recent riepen onderzoekers in Nature op tot samenwerking van clinici en neurowetenschappers om meer te weten te komen over de werking van psychologische behandelingen met het doel om ze te verbeteren (Holmes, Craske & Graybiel, 2014).

Mogelijk is er thans meer kennis over ziekteprocessen dan over processen van herstel. Kennis over ziekteprocessen is overigens relevant voor wat er kan gebeuren als je niets doet, of als wát je doet niet werkt. Er is echter ook behoefte aan kennis over welke mogelijkheden er zijn om zo vroeg mogelijk een ongunstig beloop te herkennen en te beïnvloeden. En wellicht is het nu al mogelijk veel betere resultaten te boeken als de beschikbare kennis beter wordt geïntegreerd en benut.

Recentelijk promoveerde Zandi op een onderzoek waarin zij vaststelde dat waarschijnlijk veel Marokkaanse jongeren ten onrechte zijn gediagnosticeerd als ‘schizofreen’. Het heeft ermee te maken dat de gestandaardiseerde instrumenten niet cultureel specifiek zijn. Deze studie roept vragen op bij een serie studies van onder anderen Selten. Die had vastgesteld dat psychotische stoornissen veel vaker voorkomen bij Marokkaanse jongvolwassenen. Het roept ook twijfels op over de zorg die deze mensen hebben gehad. Als de psychiaters zich aan de richtlijn zouden hebben gehouden, waren de interventies wellicht evidence-based, maar toch niet ter zake (Zandi, 2014; Selten et al., 2001).

Gezien de vele veranderingen in en om de psychische gezondheidszorg is veerkrachtig leiderschap nodig (Ledesma, 2014).
