Inleiding

Volgens de NHG-Standaard Cardiovasculair risicomanagement behoren voorlichting en counseling over een gezonde leefstijl tot de taken van praktijkondersteuners. Het is echter niet bekend of voorlichting en counseling effect hebben op de primaire preventie van hart- en vaatziekten. Als praktijkondersteuners patiënten bij het cardiovasculair risicomanagement betrekken, heeft dat dan effect op de leefstijl, het cardiovasculair risico en de risicoperceptie?

Methode

Ons onderzoek had de vorm van een clustergerandomiseerde, gecontroleerde trial in 25 huisartsenpraktijken met 615 patiënten. De interventie bestond uit risico-inventarisatie, risicocommunicatie, een keuzehulp en motiverende gespreksvoering. Controlepatiënten ontvingen een risico-inventarisatie en de gebruikelijke zorg. Na 12 weken maten we de zelfgerapporteerde risicoperceptie en ongerustheid. Na 1 jaar maten we zelfgerapporteerde uitkomsten voor leefstijl (roken, alcoholgebruik, voeding en bewegen) en achterhaalden we via de praktijkondersteuners het cardiovasculair tienjaarsrisicoprofiel.

Resultaten

Na 12 weken verbeterde in de interventiegroep de risicoperceptie en nam onterechte ongerustheid af. Na 1 jaar waren er geen significante verschillen tussen de beide groepen wat betreft leefstijl en het cardiovasculair tienjaarsrisico.

Conclusie

Intensieve begeleiding door praktijkondersteuners die zijn getraind in risicocommunicatie en motiverende gespreksvoering had een positief effect op de risicoperceptie en ongerustheid, maar geen extra effect op leefstijl en het cardiovasculair tienjaarsrisico.
