Persisterende radiuskopluxatie bij gemiste variant van de monteggia-fractuur

Samenvatting

De monteggia-fractuur is een relatief zeldzame fractuur en kan bij kinderen gemakkelijk worden gemist. Dit geldt ook voor een variant van deze fractuur, waarbij sprake is van een plastische deformiteit van de ulna, zonder fractuur. Indien dit letsel niet in de acute fase wordt onderkend, zal de radiuskopluxatie persisteren en op langere termijn klachten geven. Operatief ingrijpen is dan meestal noodzakelijk. In dit artikel wordt een casus beschreven van een gemiste variant, het type-1-monteggia-fractuur. Daarnaast wordt de literatuur beschreven. De resultaten van operatieve behandeling van chronische monteggia-fracturen lijken goed, maar bewijs hiervoor door middel van vergelijkende studies ontbreekt. Wel worden er belangrijke complicaties gemeld bij operatieve behandeling van deze fracturen.
