Samenvatting

Patiëntenvoorlichting is een planmatig proces om leefstijl- en gedragsveranderingen te bewerkstelligen. Zelfmanagement van de patiënt kan worden bevorderd door het vergroten van de participatie. Patient empowerment en een patiëntgerichte communicatiestijl zijn daarbij belangrijk. Shared-decision making is een interpersoonlijk proces waarin zorgverlener en patiënt samenwerken om aan gezondheid gerelateerde besluiten te nemen. Intervention Mapping bestaat uit zes stappen om op basis van evidence een gezondheidskundige interventie te ontwikkelen en uit te voeren. Motivational Interviewing beoogt het vergroten van de intrinsieke motivatie door de patiënt bewust te laten kiezen voor het gewenste zelfmanagementgedrag. eHealth kan patiëntparticipatie en patient empowerment ondersteunen en kan worden aangeboden in geïntegreerde zorgnetwerken, ziekenhuisinformatiesystemen en klinische informatiesystemen. De training ‘Motivering bevorderen van het zelfmanagement van de patiënt’ biedt verpleegkundigen concrete handvatten om een patiënt te motiveren tot beter zelfmanagement.
