3. Onderzoek van hart en vaten

Samenvatting

Dit hoofdstuk beschrijft het onderzoek van hart en vaten. Hierbij horen ook afwijkingen in lichaamsbouw, cyanose, en de observatie van verschillende vormen van dyspnoe. Inspectie, palpatie en auscultatie zijn alle van belang bij het lichamelijk onderzoek. De inspectie van het hart, van de arteriën en van de venen wordt besproken. De techniek van inspectie en de mogelijke bevindingen van het pulsatiepatroon van de vena jugularis worden beschreven en verklaard, waaronder propgolven en het kikkerfenomeen.
De palpatie van pols, hart, perifere arteriën, lever en milt wordt behandeld.
De auscultatie komt uitgebreid aan de orde. De keuze voor een goede stethoscoop wordt besproken. Voor vastleggen van de bevindingen bij auscultatie werd vroeger de techniek van de fonocardiografie (inclusief polscurven) gebruikt. In dit boek zijn veel fonocardiogrammen opgenomen, omdat deze een leerzame illustratie vormen van wat met de stethoscoop kan worden gehoord.
Wat zijn de oorzaken van tonen en souffles? Wanneer is een souffle onschuldig?
Alle mogelijke auscultatiebevindingen, waaronder klepafwijkingen en shunts, worden uitgelegd, geïllustreerd en in verband gebracht met anamnese en andere bevindingen bij lichamelijk onderzoek. Er zijn verwijzingen opgenomen naar websites die betrouwbare weergaven van hartgeluiden bieden.
