Mishandelde vrouwen over onthulling aan de huisarts: praten helpt echt

Doel Wij wilden onderzoeken wat vrouwen het belangrijkst vinden als zij hun huisarts vertellen over ervaren partnergeweld. We gaan na wat de gevolgen van de onthulling zijn voor de manier waarop de vrouwen de situatie hanteren.
Methode We interviewden 36 vrouwen om te achterhalen hoe zij de onthulling bij de huisarts ervaren hebben. De interviews vonden plaats binnen 4 weken na het bezoek aan de huisarts.
Resultaten De meeste vrouwen gingen naar de huisarts met een medische klacht. Slechts drie vrouwen hadden zich voor het huisartsbezoek voorgenomen om over het geweld te vertellen. Vijfentwintig vrouwen hadden de meeste waardering voor de communicatieve benadering, waarbij de huisarts empathie toont en ‘empowerment’ bevordert. Negen vrouwen gaven de voorkeur aan een instrumentele benadering, waarbij de nadruk ligt op medisch onderzoek, informatievoorziening en behandeling. Van deze laatste groep wensten acht vrouwen dat de huisarts deze aanpak combineerde met de communicatieve benadering.
Twintig vrouwen merkten op dat ze na de onthulling het gevoel hadden dat er een verandering had plaatsgevonden in de mogelijkheden om met de situatie om te gaan. De positie van deze groep vrouwen omschreven wij met de term ‘in transitie.’ Een groep van dertien vrouwen verkeerde in een toestand die wij ‘opgesloten’ noemden. Zij voelden zich niet in staat te veranderen, hadden geen controle over hun situatie en waren bang voor degene die hen had mishandeld. Drie vrouwen reageerden terughoudend op de mogelijkheid van verandering.
Conclusie Vrouwen die de huisarts over het geweld vertellen blijken een communicatieve benadering, die gepaard gaat met empathie en ‘empowerment’, te waarderen. Een ruime meerderheid van de vrouwen zag mogelijkheden om de situatie te veranderen. Praten over het geweld blijkt een belangrijk stap in het veranderingsproces van mishandelde vrouwen. Huisartsen behoren het belang van hun positie als vertrouwenspersoon te onderkennen en moeten vragen naar partnergeweld.
