Maagklachten bij de huisarts, is er wel nieuws?

Kanttekeningen bij drie richtlijnen

Samenvatting

Het verschijnen van enkele richtlijnen vormt de aanleiding het maagklachtenbeleid in de eerste lijn op een drietal punten van commentaar te voorzien.

Omtrent H. pylori blijven de meningen verdeeld. De enige goede indicatie voor eradicatie van H. pylori in de huisartsenpraktijk is een aangetoond peptisch ulcus. Naar uitbreiding van de indicaties moet meer onderzoek worden gedaan..

Bij de keuze voor een medicament om de behandeling vóór gastroscopie mee te starten, lijkt de theorie ingehaald door de praktijk: relatief snel wordt in de eerste lijn gebruikgemaakt van protonpompremmers. Hoewel deze zeker het meest effectief zijn, heeft het gebruik ervan na toepassing van lichtere middelen voordelen..

Gastroscopie wordt door huisartsen aangevraagd bij alarmsymptomen of als de behandeling bij laagrisicopatiënten faalt. Een gastroscopieaanvraag is vooral zinvol bij alarmsymptomen; bij therapiefalen levert gastroscopie relatief weinig op. Een maagfoto geeft vooral vaak aanleiding toch ook nog een gastroscopie aan te vragen en kan daarom beter achterwege worden gelaten.

Conclusie: bij maagklachten is het onderscheid tussen functionele klachten en objectieve pathologie het belangrijkste. Daarvoor zijn een goede anamnese en voldoende consulttijd nodig. Dat moet ertoe leiden dat peptisch ulcuslijden tijdig wordt onderkend en behandeld met H. pylori-eradicatie, dat uitsluitend mensen met gastro-oesofageale refluxziekte (GORZ) behandeld worden met protonpompremmers en gastroscopie vooral bij alarmsymptomen wordt aangevraagd. Een terughoudend ‘step-up’-beleid met betrekking tot de medicatie is daarvoor nog steeds een goed uitgangspunt.
