De theorie van somatische fixatie leidt tot een aantal vooronderstellingen bij huisartsen: vage onbegrepen klachten hebben vaak een psychosociale achtergrond; patiënten ontkennen die achtergrond; patiënten vragen vaak om onnodig medisch handelen en onnodig medisch handelen is schadelijk voor patiënten. Dit gedachtengoed wordt bij empirisch onderzoek inderdaad aangetroffen bij huisartsen. Patiënten met onbegrepen lichamelijke klachten hebben het gevoel niet serieus genomen en als psychisch geval beoordeeld te worden. Uit registraties van consulten van patiënten die met onverklaarde lichamelijke klachten hun huisarts bezoeken blijkt dat de vooronderstellingen van huisartsen niet kloppen. Patiënten ontkennen psychosociale oorzaken niet en het zijn niet patiënten maar huisartsen die onnodig medisch handelen veroorzaken. Huisartsen exploreren de klachten onvoldoende en schatten niet goed in waar patiënten voor komen. Zij tonen weinig empathie en medeleven waar dergelijke patiënten nu juist het meest behoefte aan hebben. Wil de huisartsgeneeskunde de pretentie van echt patiëntgericht werken waarmaken, dan zal de huisarts allereerst zijn vooronderstellingen moeten veranderen en vervolgens zijn gedrag in de spreekkamer.
