Doel Bepalen van de diagnostische waarde van anamnese, lichamelijk onderzoek, bezinking (BSE) en C-reactief proteïne (CRP) voor pneumonie; opstellen van een predictieregel om de aanwezigheid van pneumonie te kunnen schatten en het vaststellen van een laagrisicogroep die geen antibiotische behandeling nodig heeft.
Methode Vijfentwintig huisartsen in Zuid-Nederland registreerden klinische informatie en de diagnose van 246 volwassen patiënten die met een LLWI op het spreekuur kwamen.
Bloedmonsters voor BSE en CRP werden afgenomen en thoraxfoto’s (referentiestandaard) werden vervaardigd.
Resultaten Droge hoest, diarree en temperatuur van =38 ºC waren onafhankelijke en statistisch significante voorspellers van pneumonie. Demping bij percussie, crepitaties bij auscultatie en de klinische diagnose pneumonie door de huisarts hadden geen voorspellende waarde. BSE en CRP hadden hogere diagnostische odds-ratio’s dan elk gegeven uit anamnese en lichamelijk onderzoek. Het toevoegen van CRP aan het klinische predictiemodel leidde tot een significant hogere kans op een correcte diagnose.
Conclusie De meeste traditionele ziekteverschijnselen van pneumonie en de klinische diagnose van de huisarts waren niet voorspellend voor pneumonie. Droge hoest, diarree, temperatuur =38 °C, verhoogde BSE en CRP waren dit wel.
