Bij de diagnostiek bij vergeetachtigheid en andere cognitieve stoornissen moet eerst worden vastgesteld óf er wel sprake is van cognitieve stoornissen. Vervolgens sluit de huisarts behandelbare oorzaken, zoals depressie en somatische oorzaken uit en stelt hij vast of er sprake is van dementie. Is dit het geval, dan doet hij nadere diagnostiek naar comorbiditeit (somatisch en psychiatrisch), functionele beperkingen en de vorm van de dementie. Bij het vaststellen van cognitieve stoornissen en voor het differentiëren tussen de verschillende oorzaken hiervan, nemen anamnese en heteroanamnese en het afnemen van een eenvoudige cognitieve functietest, zoals de MMSE, een centrale plaats in. Met lichamelijk onderzoek en eenvoudig laboratoriumonderzoek kan de huisarts behandelbare somatische oorzaken van vergeetachtigheid opsporen. Met een CT-scan kunnen vooral behandelbare intracraniële oorzaken worden gevonden en met een MRI specifieke vormen van dementie. Diagnostiek moet niet alleen ziektegericht maar ook zorggericht zijn.
