Samenvatting

Meer dan de helft van de ouderen met kanker is kwetsbaar in de betekenis van dat er sprake is van het opeenstapelen van lichamelijke, psychische en sociale tekorten in het functioneren. Dit geeft een verhoogd risico op ongewenste uitkomsten van een (kanker)behandeling, met een negatieve impact op de kwaliteit van leven. Het is van belang een goede inschatting te hebben van de mate van kwetsbaarheid in de verschillende domeinen (somatisch, psychisch, sociaal en functioneel functioneren) om een goed medisch/sociaal netwerk rond de patiënt op te bouwen om complicaties en problemen op te kunnen vangen. Proactieve follow-up is gewenst. Daarnaast is het belangrijk om persoonlijke wensen en doelen van de patiënt te inventariseren.
