Kan een patiënt met klachten van graspollenallergie zonder voorafgaande tests behandeld worden?

Achtergrond Volgens de huidige NHG-Standaard Allergische en hyperreactieve rhinitis is op basis van anamnese de diagnose graspollenallergie met zo veel zekerheid te stellen dat zonder specifieke RAST-test gestart kan worden met behandeling. Wij hebben onderzocht of deze aanbeveling gebaseerd is op resultaten van wetenschappelijk onderzoek en of de anamnese ook voldoende zekerheid geeft over de diagnose om te kunnen starten met (sublinguale) immunotherapie.

Methoden In MEDLINE is systematisch gezocht naar onderzoeken die de resultaten vergelijken van in-vitrotests op specifiek IgE met die van de anamnese bij patiënten met een vermoedelijke gras- of boompollenallergie. De methodologische kwaliteit beoordeelden we met behulp van QUADAS. Als referentietests gebruikten we Phadebas RAST, Pharmacia Cap Systeem en/of Phadiatop; de indextest was de anamnese. We berekenden likelihood-ratio’s met 95%-betrouwbaarheidsintervallen.

Resultaten In totaal 12 artikelen konden worden geïncludeerd. De methodologische kwaliteit van de onderzoeken was slecht. De onderzoeken liepen sterk uiteen in patiëntenpopulatie en gebruikte referentietest. Geen enkel onderzoek vond plaats in een huisartsenpopulatie. In 9 van de 12 onderzoeken was de likelihood-ratio (LR) van een positieve anamnese =3. Deze LR geeft voldoende zekerheid om symptomatisch te gaan behandelen.

Conclusie De geïncludeerde onderzoeken lieten tegenstrijdige resultaten zien. Deze heterogeniteit kon niet worden verklaard door verschillen in onderzoeksopzet. De aanbeveling in de NHG-Standaard om op basis van de anamnese te starten met behandeling van een graspollenallergie gaat alleen op voor symptomatische behandeling. Voordat men start met immunotherapie is een specifieke RAST-test noodzakelijk.
