Het hepatopulmonaal syndroom: ook bij kinderen

Samenvatting

Bij een veertienjarig meisje met de ziekte van Crohn, scleroserende cholangitis en tekenen van levercirrose werd de diagnose hepatopulmonaal syndroom (hps) gesteld. Sinds een paar maanden had ze last van toenemende vermoeidheid en kortademigheid. Nader onderzoek liet zien dat ze permanent lage saturaties (tussen 0,60 en 0,70) had. Met het uitsluiten van cardiale en pulmonale oorzaken van de lage saturaties en op grond van een ventilatie-perfusiescan kon de diagnose hps worden gesteld. Vier maanden na het stellen van de diagnose werd levertransplantatie verricht. Twee maanden daarna was de saturatie genormaliseerd. De diagnostiek van hps is moeilijk wegens de weinig specifieke klachten en het vaak sluipende beloop. Bij kinderen wordt de diagnose hierdoor vaak (ten onrechte) niet overwogen. Levertransplantatie is de enige beschikbare behandeling. Vroegtijdige onderkenning is van groot belang wegens de vaak snelle progressie van het syndroom en de grotere kans op regressie bij tijdige levertransplantatie.
