Inleiding Huisartsen schrijven nog steeds te veel antibiotica voor bij acute rhinosinusitis. Wij verrichtten een individuele patiënten data meta-analyse (IPDMA) van gerandomiseerde, placebogecontroleerde onderzoeken om een subgroep van patiënten te identificeren die wellicht wel baat heeft bij antibiotica.
Methoden We controleerden en analyseerden opnieuw de individuele patiëntengegevens van 2547 volwassenen uit 9 onderzoeken. Wij bepaalden het effect van behandeling met antibiotica en de prognostische waarde van symptomen en klinische bevindingen. De uitkomsten zijn weergegeven als het aantal patiënten dat moet worden behandeld om een extra patiënt te genezen (number needed to treat - NNT).
Resultaten Vijftien patiënten met rhinosinusitisklachten moeten worden behandeld met antibiotica om 1 patiënt extra te genezen (95%-BI NNT [voordeel] 7 tot NNT [schade] 190). Patiënten bij wie de dokter pus in de keelholte ziet, genezen eerder met antibiotica, maar men moet nog steeds 8 van dit soort patiënten behandelen om 1 extra patiënt te genezen (95%-BI NNT [voordeel] 4 tot NNT [schade] 47). Bij oudere patiënten en bij patiënten met langduriger of ernstiger symptomen duurt de genezing langer, maar zij hebben niet meer baat bij behandeling met antibiotica dan anderen.
Beschouwing Patiënten met rhinosinusitisklachten hebben geen baat bij antibiotica. Wij konden aan de hand van de gewone symptomen en klinische bevindingen geen subgroep van patiënten identificeren die wel baat zouden hebben bij antibiotica. Het is onnodig om patiënten met antibiotica te behandelen, ook niet als de klachten langer dan 7-10 dagen bestaan.
