Samenvatting

Wanneer een patiënt met algemene gewrichtsklachten voor de eerste keer bij de arts komt, is het zinvol zo snel mogelijk een diagnose te stellen, omdat vroege behandeling belangrijk is bij reumatoïde artritis en septische (ofwel bacteriële) artritis teneinde ernstige schade te voorkomen. Bij verdenking op deze aandoeningen is een acute verwijzing voor verder onderzoek dan ook noodzakelijk. Het is belangrijk onderscheid te maken tussen inflammatoire aandoeningen (artritis) en niet-inflammatoire gewrichtsaandoeningen (artrose). Bij inflammatoire aandoeningen gaat het meestal om niet-bacteriële aandoeningen zoals om systeemziekten (systemische lupus erythematodes, polymyositis, syndroom van Sjögren, sclerodermie), spondyloartritis (o.a. ziekte van Bechterew, reactieve artritis), jicht en pseudojicht, en acuut reuma. Bij een eerste bezoek aan de arts van een patiënt met gewrichtsklachten leveren de voorgeschiedenis, anamnese en het lichamelijk onderzoek de belangrijkste bijdragen aan de diagnostiek. De voorspellende waarde van laboratoriumonderzoek bij onduidelijke gewrichtsklachten is zeer beperkt. Acuut optredende gewrichtspijn met koorts is een alarmsymptoom dat kan wijzen op een septische artritis.
