Samenvatting

Nekklachten komen veel voor. Meestal betreft het aspecifieke nekpijn. Als eenmaal helder is waarom juist deze patiënt de moeite neemt om hem hiervoor te consulteren, is het voor de arts een stuk eenvoudiger bij aspecifieke nekpijn uit te leggen dat verder onderzoek vaak niet nodig is. Oorzaken van specifieke nekklachten zijn bijvoorbeeld nekhernia (discusprotrusie), whiplash, spondylolisthesis en cervicale artrose. De grote lijn in de diagnostiek van nekpijn is het uitsluiten van specifieke oorzaken middels een goede anamnese. Wees alert op maligniteiten (botmetastase), subarachnoïdale bloeding, meningitis (bij kinderen met een plotseling begin van een stijve nek), reumatoïde artritis en torticollis. Vervolgens volstaat een gericht lichamelijk onderzoek. Daarbij wordt vooral gekeken naar beperkingen van de mobiliteit. Aanvullende diagnostiek is nauwelijks nodig. Verder is het zinvol op basis van bekende prognostische factoren alert te zijn op dreigende chroniciteit en hieraan in het onderzoek apart aandacht te besteden.
