Samenvatting

Gewichtstoename is in tegenstelling tot overgewicht een klacht waarvoor geen objectieve criteria bestaan. Vooral vrouwen van middelbare leeftijd komen met deze klacht bij de huisarts. Het doel van de diagnostiek is afhankelijk van de hulpvraag. Bij de diagnostiek is het zinvol onderscheid te maken tussen gewichtstoename door vet of andere oorzaken, zoals coronaire hartziekten, hypertensie, diabetes mellitus, galblaasaandoeningen of metabool syndroom. In overgrote meerderheid worden bij de klacht gewichtstoename door vet echter geen lichamelijke oorzakelijke afwijkingen gevonden. Indien anamnestisch hiervoor geen aanwijzingen zijn, kan het onderzoek zich verder richten op de energiebalans: de intake en het verbruik van calorieën. Omdat de meeste mensen met gewichtstoename ook lijden aan overgewicht, is het zinvol ook aandacht te besteden aan de risico’s van het overgewicht, vooral het risico op hart- en vaatziekten. Voorbeelden van andere aandoeningen die door adipositas worden bevorderd zijn diabetes mellitus, maligniteiten, cholelithiasis en pancreatitis, leverafwijkingen, refluxoesofagitis, fertiliteitsproblematiek, incontinentie, respiratoire problemen, artrose, veneuze insufficiëntie en varices.
