De diagnose fibromyalgie is controversieel door een gebrek aan causale verklaringen en geschikte behandelingen. Toch hebben patiënten met chronische gegeneraliseerde pijnklachten zorg en aandacht nodig.

Heldere gegevens over de prevalentie en prognose van fibromyalgie in de eerste lijn ontbreken. Wij bespreken de diagnostiek bij patiënten met chronische gegeneraliseerde pijn, waarbij voor spierkrachtmeting en gering bloedonderzoek gekozen kan worden.

Veel strategieën zijn onderzocht, maar geen enkele vormt de ideale behandeling van fibromyalgie. Activerende behandeling is effectief gebleken voor het algemeen welbevinden van patiënten met fibromyalgie. Cognitieve gedragstherapie lijkt de meest aangewezen behandeling, maar moet verder onderzocht worden.
