Epidemiologie: prevalentie, incidentie en prognose

Abstract

Het toenemend belang van hartfalen voor de volksgezondheid blijkt onder meer uit de grote aandacht die het syndroom krijgt in de lekenpers, op wetenschappelijke congressen, in medische tijdschriften en tijdens vele nascholingen. Deze aandacht versterkt het idee dat er een epidemie van hartfalen aan het ontstaan is en ook de titels van overzichtsartikelen, zoals ‘Heart failure – an epidemic of uncertain proportions’, suggereren dat het voorkomen van hartfalen epidemische proporties heeft aangenomen.13 Nog steeds vormen hart- en vaatziekten de belangrijkste doodsoorzaak in geïndustrialiseerde landen en het aandeel van hartfalen hierin neemt toe. Hartfalen heeft bovendien een nadelige invloed op de kwaliteit van leven, onder andere door de verminderde inspanningstolerantie, de frequente ziekenhuisopnamen en mogelijke bijwerkingen van het grote aantal, overigens effectieve, geneesmiddelen dat de patiënten gebruiken. Naar schatting wordt in Nederland momenteel ruim 1 procent van het budget voor de gezondheidszorg besteed aan hartfalen. De verwachting is dat het belang van hartfalen voor de volksgezondheid verder toeneemt, vooral als gevolg van de veroudering van de bevolking en de verbeterde overleving van patiënten met coronaire hartziekten – de belangrijkste oorzaak van hartfalen in westerse landen.4 De ondertitel van het overzichtsartikel in het New England Journal of Medicine, ‘an epidemic of uncertain proportions’, illustreert ook het gebrek aan goede schattingen van de precieze omvang van de ‘epidemie’.1 In de afgelopen decennia heeft het wetenschappelijk onderzoek naar hartfalen een grote vlucht genomen en het aantal therapeutische (mega)trials is opvallend groot te noemen, al is het aantal de laatste jaren afgenomen.
