Effectiveness of innovations in nurse led chronic disease management for patients with chronic obstructive pulmonary disease: a systematic review1)

Samenvatting

De belasting voor patiënten die wordt veroorzaakt door gezondheidsproblemen die samenhangen met copd (Chronic Obstructive Pulmonary Disease), hebben ertoe geleid dat er veel vernieuwingen zijn doorgevoerd in de klinische zorg voor deze patiënten. De literatuur met betrekking tot deze innovaties beschrijven in hoofdzaak twee soorten: 1) substitutie van ziekenhuiszorg in de thuissituatie of vroeg ontslag na acute exacerbaties van copd en 2) interventies die erop gericht zijn om het disease management van copd als chronische aandoening te verbeteren. Dergelijke interventies worden doorgaans geleid, gecoördineerd en verzorgd door verpleegkundigen.
