Op een maandag in februari presenteert mevrouw Peeters zich op het open avondspreekuur, dus zonder afspraak. Traditioneel is dit relatief druk. Net op dit moment zijn er computerproblemen waardoor de elektronische dossiers niet te raadplegen zijn. De patiënte wordt gezien door de huisarts in opleiding (haio), die net een week bezig is op de praktijk. Het gaat om een kort consult – en dat is wel meegenomen in dit drukke spreekuur. De 31-jarige patiënte geeft aan dat ze enkel voor herhalingsvoorschriften van haar huidige medicatie komt: duloxetine (Cymbalta®) en quetiapine (Seroquel®). De haio informeert kort naar de omstandigheden, die omschreven worden als gewoon een herhalingsvoorschrift voor onderhoudsmedicatie, vraagt niet verder door naar de omstandigheden en schrijft daarop het gevraagde voorschrift met de hand, waar dit normaal afgedrukt wordt via het automatisch dossiersysteem, gezien de voorgenoemde computerpech. …
