Diclofenacgel bij gonartrose

Vraagstelling Paracetamol is vanwege het brede veiligheidsprofiel de eerste keus bij de medicamenteuze behandeling van gonartrose.1 NSAID’s zijn tweede keus. Bij NSAID’s wordt terughoudendheid geadviseerd bij ouderen, bij patiënten met nierinsufficiëntie, hypertensie, hart- en vaatlijden en bij patiënten die antistollingsmiddelen gebruiken.1 De NHGStandaard Niet-traumatische knieproblemen bij volwassenen vermeldt dat topicale NSAID-gels minder gastro-intestinale bijwerkingen hebben, maar waarschijnlijk kortdurend effectief zijn.1 In de gerefereerde meta-analyse wordt echter geen onderscheid gemaakt in locatie van de artrose.2 Dit kan geleid hebben tot een onderschatting van het effect aangezien de biologische beschikbaarheid per locatie kan verschillen.3 Momenteel is in Nederland alleen diclofenacgel als geregistreerd medicijn verkrijgbaar. Is diclofenacgel even effectief en heeft het minder systemische bijwerkingen als orale diclofenac bij pijn door gonartrose? …
