Samenvatting

Deze studie is uitgevoerd om de huidige gang van zaken in Nederland te inventariseren wat betreft diagnostiek, behandeling en follow-up van anterieure urethrastricturen, waartoe een 16-item vragenlijst is toegestuurd aan alle 323 urologen.
Het responspercentage bedroeg 74%. 97% van de urologen gebruikte een urethrotomia interna en 23% een urethraplastiek. 6% van de urologen verrichtte > 5 urethraplastieken per jaar; bij urologen jonger dan 50 jaar was dit 3%. Endoscopische procedures werden veelvuldig toegepast, ook bij zeer hoge recidiefkans. De urethraplastiek werd door 21% van de urologen gezien als primaire behandeloptie. Diagnostische work-up en wijze van follow-up, evenals de definiëring van succes die deze behandeling met zich meebrachten, was zeer divers.
De meeste urologen menen dat een urethraplastiek alleen een optie is na falen van een urethrotomia interna. Endoscopische procedures worden veelvuldig toegepast, ook bij een grote kans op recidief. Urethraplastiek wordt slechts door een klein aantal, vooral oudere urologen frequent verricht. Centralisatie en gecentraliseerde opleidingsvoorzieningen zijn nodig om in de toekomst hoogwaardige reconstructieve zorg te kunnen bieden.
