6. Behandeling op de intensive care

Samenvatting

Ernstig traumatisch hoofd-/hersenletsel (THL) is wereldwijd een toenemend medisch en sociaal probleem, met meer dan 10 miljoen slachtoffers per jaar die worden opgenomen in het ziekenhuis. Patiënten met ernstig THL worden opgenomen op de intensive care (IC) voor bewaking om secundaire hersenschade te voorkomen. Centraal in de behandeling staat het verminderen van intracraniële druk (ICP) en het waarborgen van de cerebrale bloeddoorstroming (CPP) van de hersenen. Hierbij wordt gebruikgemaakt van een stapsgewijze intensieve behandeling. Het ontwikkelen van een verhoogde ICP als teken van toenemend intracranieel volume dient snel herkend en behandeld te worden gezien de associatie met morbiditeit en mortaliteit. Invasieve monitoring van de ICP/CPP is momenteel de enige continue en betrouwbare meetmethode die voorhanden is. Nieuwe ontwikkelingen met andere vormen van monitoring op de IC worden onderzocht en de verwachting is dat de behandeling van ernstig THL-patiënten op de IC in toenemende mate geïndividualiseerd zal gaan worden.
