5. Addendum: discusdegeneratie van de lumbale wervelkolom

Samenvatting

Dit hoofdstuk beschrijft de anatomie, fysiologie en pathologie van de discus intervertebralis. Aan bod komen de manier waarop de discus zijn voeding krijgt, hoe hij geïnnerveerd wordt en welk typen letsel in de loop van een mensenleven kunnen ontstaan. Verder beschrijft het hoofdstuk de consequenties van een discusdegeneratie voor omringende weefsels, zoals de eindplaten van de wervellichamen, de facetgewrichten en de weefsels binnen het wervelkanaal. Begrip van deze mechanismen is van belang voor een juiste diagnose na de anamnese en het klinisch onderzoek van een rugpatiënt.

Innervatie vindt plaats via de ramus ventralis en van de nervus sinuvertebralis (ook wel nervus recurrens genoemd) van het betreffende segment, maar ook van de vertakkingen van de daarboven en daaronder liggende segmenten.

Extrasegmentale pijn is pijn die wordt gevoeld in meerdere huidsegmenten.

Hetzelfde mechanisme wordt aangetroffen bij een gedegenereerde achillespees. Men vermoedt dat ook hier pijn tot stand komt door prikkeling van de zenuwen die de prolifererende bloedvaatjes begeleiden.

Collageen type I is goed bestand tegen trekbelasting. Het vormt een belangrijk bestanddeel van pezen, gewrichtsbanden en kapsels.

Bewegingssegment: hiermee wordt de discus bedoeld inclusief de beide aangrenzende wervels.

De terminologie is nogal verwarrend: soms wordt een lichte uitpuiling zonder anulaire rupturen al een hernia genoemd. Meestal wordt de term ‘hernia’ gebruikt als door extrusie van nucleair materiaal een of meer zenuwwortels worden gecomprimeerd en hierdoor uitvalsverschijnselen bestaan. Het nucleaire materiaal is bij een extrusie buiten de discus getreden.
